The multi-disciplinary team works with people over the age of 65 with both functional and organic mental health issues. Care coordinators within the team work as independent practitioners to enable and empower those experiencing  mental health problems and offering support to  their carers. 

The team is uniquely linked with the Croydon Memory Service, the Croydon Home Treatment Team and the Croydon Older Adult Liaison Team. There is excellent working relations with Croydon Older Adults  Home Treatment Team.

The CMHT is  based at  Selhurst Road with excellent transport links nearby. In the role of Care Coordinator, we  will be encouraging you to work as an autonomous practitioner, with our client group  to help maintain and support their mental wellbeing in the community.

Working for the Mental Health of Older Adult and Dementia, Clinical Academic Group (CAG) ensures personal and career development by:

• Attending collaborative staff development forums and Nursing Councils, allowing staff to network with senior management of both the Directorate and Trust level

• Supporting staff with further education opportunities

• Encouraging staff to actively participate in Research with the IOPPN
