We are seeking to appoint a skilled and enthusiastic Primary Care Mental Health Liaison Practitioner to be part a new service, Primary Care Mental Health Service (PCMHS) - Lewisham.
PCMHS is an exciting development in mental health service provision in Lewisham. The service aims to provide proactive, strength-based care for service users experiencing mental health problems in a primary care setting using a stepped care/ wrap around model.
The service provides care and support to service users who require a level of care above that available through general medical services, but do not require secondary care mental health intervention. PCMHS also support service users who have on-going, low support needs and those who are transitioning from secondary care mental health services to primary care services. The service collaborates with the single point of access to mental health service and participates in the triaging referrals to appropriate services. PCMHS provides advice, consultation and training to primary care staff and liaison link working with the GP neighbourhoods.
This is a new integrated team which incorporates new caring roles in social prescribing and care navigation, drawing on the expertise of third sector partner providers. The team comprises mental health advisors, peers support workers, nurses, social workers, consultant psychiatrist and other workers.
You will be a nurse, social worker or occupational therapist with experience of working across the primary and secondary care interface, and with an ability to balance caseload commitments. You will have sound clinical knowledge, good assessment, decision-making and supervisory skills. You will be required to participate in triaging of referral, assessments, managing a caseload and providing clinical supervision to other staff members. Having an area of specialism/ special interest will be desirable, and you will work as a link person for designated GP surgeries in the local GP neighbourhoods. As a Primary Care Mental Health Practitioner, you will be part of a multi-disciplinary which includes third sector mental health workers.
The Trust provides opportunities for specialist supervision, service development and evaluation and service oriented research. There are peer and group supervision opportunities, and specialist training is available in the Trust and through Kings College, London.
South London and Maudsley (SLaM) has the widest range of Mental Health Services in the UK and also substance misuse services for people addicted to drugs and/or alcohol. It has over 230 services including inpatient wards; outpatient and community services with about 4,800 staff and serve a population of about 1,1m. It provides inpatient care for approximately 5,300 people each year and treats more than 45,000 patients in the community across Lambeth, Lewisham, Southwark and Croydon.
You must have appropriate UK professional registration.
