This is a fantastic opportunity to work as a Advanced Practitioner across the Acute and Crisis pathways in Lewisham, Lambeth, Croydon and Southwark. The post is open to qualified mental health nurses, social workers or occupational therapists. You will be borough based and will have an opportunity to state your preferred borough if shortlisted for interview.
You will provide a rapid assessment to patients within the Emergency Department and general hospital wards where admission is being considered. The aim of the role is to ensure that any unnecessary admissions are avoided and all least restrictive options have been comprehensively explored. You will also provide timely assessments to patients who are admitted to an acute psychiatric inpatient ward to ensure that discharge is facilitated at the earliest opportunity. You will be based within the local Home Treatment Team and will need to have a good understanding of how acute and crisis services interface.
This is a diverse and multifaceted role and you will need to have substantial post-registration experience in relevant care settings. You will need to promote a culture of positive risk taking and be able to challenge decisions that are not in the best interests of service users. You will need to demonstrate excellent communication ability across a wide range of professionals and services.
You will be expected to work a 24 hour shift pattern which aligns to the Home Treatment Service. The early shift is 08:00–16:00, late shift 14:00–22:00 and the night shift 21:45–08:15.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
